online duels and permission

How do I deny permission to an online duel when the other player has the first hit? Did I press something to give permission and not realize? I do tend to speed play, so maybe I doubled up on a keypress or something, but I don't think I even SAW a line of text indicating that I had the option to give permission.

I just got schooled by someone leaps and bounds above me, without my consent (as far as I can tell) and I'm CHOKED because I know this is contrary to the rules. I had 164ish hitpoints and a SINGLE HIT from this player cost me about 3x that.

Neither red realm nor special require permission before initiating a duel.

But if you were green or blue and that happened you wouldn't be able to stop it if they got first hit correct?
I haven't had that come up. No one has attacked me without permission.
